Free and Reduced School Meals Family Application Form

Free and Reduced School Meals Family Application form and directions for the 2012 – 2013 school year has been mailed.

We can ONLY accept ONE application per household and ALL household members must be listed on the application.

Please return the application by August 24, 2012 to Lynn Pylak, at the Hazel Park Schools Administration Building, 1620 E. Elza, Hazel Park, MI  48030, during the summer for processing.  Any applications completed after the August 24^th date should be returned to your child’s school or sent with the student on their first day of school.  Please remember that only one application per household is accepted.
